The Client Questionnaire is designed to consolidate your important information and facilitate our understanding of you and your goals. We realize that we may request more information than other planners, but we believe the extra time you spend completing the form and collecting the information will show in the final product.
The Financial Fitness Survey is designed to make you think about different aspects of your life and help identify those areas where you could benefit from working with a financial planner.
If it is your intention to get the best outcome please invest the time in completing this form. All information provided is in the strictest confidence. Any plan, advice or recommendations are always based on the information you provide. Too little, or inaccurate information will lead to inferior results.
The Engagement Agreement spells out for you ahead of time, how we operate, providing full disclosure, so there are never any surprises. This form would be completed prior to our engagement.
The Privacy Statement and Conflict of Interest Statement are again to fully inform you of our policies. Full disclosure is always the best procedure.
Everyone should carry an Emergency ID Information Card. We have a card that you can fill out and carry in your wallet at all times.
